The 24th Annual HomeCare & Hospice Foundation Tree of Life is a special way for you to remember your loved one’s life this holiday season or honor a special someone.
A beautifully designed, pewter-finish Sled, with engraved name and dated 2019, will adorn one of the six Trees of Life. The ornament will become a welcomed addition to your family’s permanent collection. The price per ornament is $20, plus $2.50 shipping and handling per ornament (if you choose to have your ornament mailed directly to you or a family member).
Orders for engraved ornaments must be postmarked by November 22nd. Online Sales must be ordered by November 28th. These dates will help to ensure availability of the ornaments for the Tree of Life events.
As an addition to your ornament, or perhaps as an alternative, memory tags with a loved one’s name, quote, or special memory can be purchased for $5.00 plus $2.50 shipping and handling if you choose to have it mailed.

You can choose to support a specific Tree of Life location. The following support options are available:
$500: (Rejoice Level) To include memory section in the event program, social media post, signage at the event, and recognition in the Quarterly At-Home Publication. Three ornaments — engraved with your specific requirements.
$250: (Remembrance Level) To include memory section in the event program, signage at the event. Two ornaments — engraved with your specific requirements.
$100: (Reflect Level) To Include signage at the event. 1 ornament — engraved with your specific requirements.
