RN (Registered Nurse) Care Coordinator/Clinic Contract RN



Under the supervision of the assigned clinical supervisor and in accordance with the clinic contractual agreement, the RN Care Coordinator/Clinic Contract RN is responsible for, but not limited to, assisting with clinic operations and the provision of skilled nursing services, including implementation of nursing care plans and completing assigned assessments visits. The RN Care Coordinator/Clinic Contract RN will comply with agency, PACE and Article 28 clinic regulations and policy and procedures.




  1. Assists with clinic operations, collaborating with clinic and PACE officials to assure compliance with Clinic Article 28 and agency regulations.
  2. Ensures delivery of services within the parameters defined by the patient plan of care and agency policy and procedures.
  3. Assists with performing Uniform Assessment System for clinical eligibility of participants as applicable.
  4. Assists Home Care Coordinator and/or Sr. RN Care Coordinator/Clinic Contract RN with overflow of participant care as requested.
  5. Ensures delivery of skilled nursing services within the parameters defined by the patient plan of care and agency policy and procedure.
  6. Completes participant care or assessments as necessary and develops and updates the plan of care and required documentation.
  7. Compiles and uses records, reports and statistical information for evaluation, quality improvement and planning for services.
  8. Maintains timely and quality documentation in the electronic medical record (EMR) of all clinical and nursing services provided.
  9. Makes recommendations in the development and revision of the Plan of Care to members of the interdisciplinary team (IDT).
  10. Serves as subject-to-call nurse on a rotating basis as assigned.
  11. Assists in volunteer or staff training and inservices as needed.
  12. Assists with regional and community referral sources, vendors and contracted service facilities/organizations, working diligently to ensure that a strong sense of collaboration and team building is evident.
  13. Prepares required QI reports and completes all patient documentation in a timely manner, in accordance with agency defined parameters.
  14. Maintains clinical excellence, consistently advancing knowledge base and clinical skill sets.
  15. Adheres to and reflects organizational values in daily work.
  16. Serves on agency committees as may be assigned.
  17. Maintains an obligation to report wrongdoing/violation of agency policies, applicable federal, state, and local laws, and rules and regulations pertaining to agency operations, to immediate supervisor or identified compliance officer.
  18. Completes all mandatory inservice education programs and completes any other additional in-service hours that are minimally required for the position.

  1. Current license and registration to practice nursing as a registered professional nurse in the State of New York.
  2. A minimum of two years full time or equivalent of recent applicable nursing experience required.
  3. Ability to lift a minimum of 20 pounds, perform one person transfer under routine and emergency situations, reposition a bedbound patient alone, and able to sit, stand, bend, stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities.
  4. Effective oral and written communication skills.
  5. Acceptable driver’s license and automobile available for use during work hours with current insurance
  6. Availability by telephone
  7. Successful completion of NLN proficiency medication administration exam.
