Total Senior Care Aide


Under the supervision of the Center Manager (RN) and day-to-day direction by the Total Senior Care Lead Aide, provides assistance and attendant care to participants in the Total Senior Care day center.   Provides assistance with personal care, food service, recreational programming, activities of daily living (ADL’s) and instrumental activities of daily living (IADLs).  Demonstrates the knowledge and skills necessary to participate in providing service to frail elderly participants according to assigned responsibilities and care plan.  Adheres to the care plan and receives guidance, direction and supervision appropriate ot the care setting.  Provides participants with a continuity of quality care while at the day center.



  1. Provides basic personal care assistance and/or supervision (bathing, grooming, toileting, feeding) and assistance with any other Activities of Daily Living (ADLs), including bathing in tub, and/or shower, transferring participant on and off commode and toilet, care of teeth and mouth, grooming of hair, shampooing, shaving filing nails and eating and dressing needs.
  2. Provides physical assistance and attendant care to participants attending the day center.  Assists with wheelchair and ambulatory transfer, and center and van transfer.  Provides assistance as directed by care plan with nursing care, restorative therapies, activities and meals across care settings.
  3. Assists with ambulation, moving from/to chair or wheelchair, and with walking (transfer of weight-bearing and partial weight-bearing person, pivot transfers, sliding board and hover lift transfers, pushing and lifting wheelchair with participant in it, as determined by physical and/or occupational therapist.
  4. With guidance from the IDT per care plan, follows medical recommendations such as increased physical activity, increased rest with elevation of legs, use of oxygen, etc.
  5. Provides skin care with intact skin (non-prescription and prescription applications/lotions when directed by nursing).
  6. Applies: job stockings, TED stockings, assistive devices.
  7. Assists in preparation, serving, feeding and cleaning up after congregate meals and snacks, both regular and special diet.
  8. Sets up and operates recreational, health care and other equipment.
  9. Assists with developing and conducting age and cognitively appropriate activities as planned by the Activities Coordinator (games, crafts, discussions, field trips, exercise).
  10. Assists in keeping Center in clean and orderly condition, including showers, bathrooms, kitchen and activities areas.
  11. Performs other pertinent care functions as assigned and demonstrated by the Center Manger/RN and TSC Lead Aide.
  12. Participates in assessing, developing and delivering plan of care for and documenting care for frail elderly participants as contributing member of interdisciplinary team.
  13. Assists in providing a comprehensive plan of care to meet the needs of frail elders to promote optimum health and maximizing independence and self-care in accordance with Total Senior Care philosophy.
  14. Notes changes in participants’ physical or emotional health and communicates this information to the appropriate staff person.  Reports changes in participant’s abilities for self-care to supervisor and interdisciplinary team.
  15. Knows and adheres to the philosophy and goals of Total Senior Care.
  16. Maintains a professional and safe environment for participants, visitors and staff.
  17. Keeps confidentiality of participant records, reports and discussions.
  18. Advises the Center Manager or Program Officer in ways and means to establish better accountability of Total Senior Care services to participants and referral sources.
  19. Attends and participates in scheduled staff meetings and participant care meetings as requested.
  20. Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload in order to fulfill responsibilities.
  21. Utilizes supplies and equipment economically.
  22. Informs the Center Manager or Director of Clinical Services of “unusual occurrences”, grievances and service delivery problems.
  23. Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Center Manager or TSC Lead Aide as appropriate; responds in a positive manner to constructive criticism; serves as a role model for students and staff members.
  24. Adheres to accepted infection control practices and participates in Quality Assessment Performance Improvement activities.
  25. Adheres to and reflects organizational value in daily work.
  26. Maintains an obligation to report wrongdoing/violation of agency policies, applicable federal, state and local laws, and rules and regulations pertaining to agency operations, to immediate supervisor or identified compliance officer.
  27. Completes all mandatory inservice education programs and completes any other additional inservice hours that are minimally required for the position.

  1. High School diploma or equivalent.
  2. Completion of a personal care aide training program approved by the State of New York or approved equivalency test.  Alternate training includes completion of a home health aide program or a certified nurse’s aide program.
  3. Experience as a homemaker, personal care aide, home health aide or nursing assistant is desirable.
  4. One year experience working with the frail elderly.
  5. Ability to assist with personal care at the center, including wheelchair and ambulatory transfer between center and van.  Ability to lift a minimum of twenty (20) pounds, perform one person transfer under routine and emergency situations, and able to sit, stand, bend, stoop, kneel, pull, climb, reach, lift, and perform repetitive movements of the upper extremities.
  6. Ability to demonstrate skills and equipment competencies upon hire, and annually thereafter.
  7. Interdisciplinary teamwork, ability to work effectively with culturally, economically and educationally diverse populations, and form positive inter personal relations in dealing with a wide range of staff and participants essential.
  8. Acceptable driver’s license and automobile available for use, with current insurance, preferred.
