What is a referral?

Do you know someone who is suffering from an illness or who has a disability?  Do you have a friend or relative recovering from surgery or a neighbor who can't do the things he/she once did?  Do you know someone caring for an ill child, parent or grandparent, who needs a break?  

If you think they could benefit from our services, refer them to HomeCare & Hospice.  It gives them a way to find out more!

  • A referral from a patient's physician usually means the patient already understands and desires our services.

 

  •  Anyone can make a referral for home care or hospice services. When family members, friends or neighbors make referrals, we must follow up with the patient.  It is best to know if they are interested before contacting us.

 

  • Once a referral is made, we call the patient.  If they would like to receive services, with their permission, we contact their physician.  A patient cannot be admitted for licensed home care or for hospice services without doctor's orders.

 

  • If the doctor certifies the patient eligible, we begin the enrollment process.  Trained staff work with the patient, family and physician along the way.
